"How am I driving?" survey is created.

Question 1:

How do you rate the driver's driving skills from 1 to 5 with 5 being expert?

Collect response via voice or keypad (dtmf) from the caller. Restrict the response type to "digits".

Question 2:

What is the color of the car or van?

Restrict the response type to "custom" and add possible answers.

Caller can now say Red, Blue, Grey, Green or Yellow when prompted.
